Insurance and Referrals

We accept Medicare assignment and participate in many of the region’s health insurance plans. If you have questions about our participation in your health insurance plan, please call our office at (217) 321-2020. Please bring your health insurance cards with you to your first appointment, so that we will have the information required to submit insurance claims for you.

In order to obtain the benefits you are eligible for, from your health insurance coverage, please contact your Health Insurance Company at least two weeks before your appointment. Ask if you need a referral letter from your primary care physician prior to your visit. It is your responsibility to obtain the required referral authorization prior to your visit. In addition, many health plans require preauthorization before any type of surgical procedure.

If your health insurance requires a co-payment, it will be collected at your appointment. Our office will bill your insurance directly for our services. After your health insurance plan(s) has paid their portion of the service charges, we will provide you a bill for the balance. If the cost of your eye care proves a financial burden, our office will be glad to work with you to arrange a mutually agreeable payment plan. Our office accepts checks and credit card payment through MasterCard and Visa.
